Actínica con terapias fotodinámicas y cirugía plástica

El cáncer de piel no melanoma (CPNM) se trata tradicionalmente con escisión quirúrgica. Los métodos no quirúrgicos como la crioterapia y la quimioterapia tópica, entre otros tratamientos, son otras opciones. La queratosis actínica (QA) se consideran lesiones precancerosas que con el tiempo pueden progresar a carcinoma de células escamosas (CCE). La terapia fotodinámica (TFD) ofrece un tratamiento eficaz para los QA y también es eficaz para el carcinoma basocelular superficial (CCB). El CCB nodular y la enfermedad de Bowen (CCE in situ) han mostrado tasas de respuesta aceptables con la TFD, aunque las tasas de recidiva son más altas para estos dos subtipos de CPNM. La PDT de metilaminolevulinato (MAL) es una opción de tratamiento más eficaz que la PDT de ácido 5-aminolevulínico (ALA) para el CCB nodular. Varios estudios han demostrado que la TFD da como resultado resultados cosméticos superiores en comparación con el tratamiento quirúrgico. En general, la TFD es bien tolerada, siendo el dolor el efecto secundario más común.
