Introducción
Los estudios demuestran que la causa princi-
pal de mortalidad en el intra y postoperatorio
es la presencia de la tríada: acidosis, hipo-
termia y coagulopatía. Rotondo y col. descri-
bieron una nueva forma de enfrentar a estos
pacientes a fin de disminuir la mortalidad, la
denominada Cirugía de Control de Daños
(Damage Control). La acidosis se explica por
una disminución de oxígeno por una impor-
tante pérdida sanguínea que lleva a hipovo-
lemia e hipoperfusión celular. La presencia
de hipotermia se explica por la hipovolemia,
la pérdida de calor a través del peritoneo du-
rante una laparotomía, la reanimación con lí-
quidos a temperatura ambiental (siendo más
fríos que la temperatura corporal), la transfu-
sión sanguínea, la temperatura ambiental del
quirófano, entre otros. La hipotermia produce
trastorno de la cascada de la coagulación y
disfunción de las plaquetas. (Ortega-Checa
& Vojvodic-Hernández, 2021)
El término "control de daños" se heredó de
la Marina de los Estados Unidos donde se
empleaba para los equipos especiales res-
ponsables de mantener a flote un barco
gravemente dañado hasta que era traslada-
do al puerto para su reparación definitiva.
De manera similar, la “cirugía de control de
daños” sirve para atender condiciones que
ponen en riesgo la vida de forma inminente
(mantener al paciente a flote), mientras que
el tratamiento definitivo de las lesiones que
no ponen en peligro la vida se demora has-
ta después de una reanimación adecuada.

La cirugía de control de daños (CCD) es
un recurso importante en pacientes inesta-
bles y potencialmente graves. A pesar de
que sus indicaciones iniciales estuvieron
orientadas al paciente politraumatizado, ac-
tualmente esta estrategia quirúrgica es em-
pleada también en determinados pacientes
con patología quirúrgica urgente de origen
séptico no traumático que condiciona una
situación crítica de inestabilidad hemodiná-
mica. (Camacho Marente et al., 2019)
La cirugía de control de daños se divide en
tres etapas: la primera de ellas es una ciru-
gía inicial, control de la hemorragia y la con-
taminación; la segunda, está compuesta
por la reanimación fisiológica del paciente
en la unidad de cuidados intensivos; mien-
tras que la tercera y última consta de una ci-
rugía definitiva o re-exploración. Estas tres
fases o etapas conforman el procedimiento
completo. La cirugía de control de daños se
basa en el paciente severamente traumati-
zado. En la actualidad es uno de los proble-
mas más complejos a los que se enfrenta un
cirujano en la práctica diaria, sin embargo,
con la aplicación adecuada de este proce-
dimiento se ha llegado a mejorar la supervi-
vencia de los pacientes que son sometidos
a la misma. (Alonzo & López, 2020)
Asensio establece un protocolo para definir
las indicaciones todavía más precisas, el re-
querimiento de más de 4 litros de concentra-
dos de hematíes, o si se emplea una com-
binación de hematíes y sangre total de más
de 5 litros, o el volumen total de fluidoterapia
administrada supera los 10-12 litros; o cuan-
do el paciente presenta un pH menor de 7,2,
una temperatura menor de 34°C o una pér-
dida sanguínea estimada mayor de 5.000
ml; una lesión venosa mayor inaccesible
o una lesión mayor concomitante fuera del
abdomen; la necesidad de procedimientos
quirúrgicos prolongados en un paciente con
respuesta subóptima a la reanimación o in-
capacidad para lograr la hemostasis debida
a la coagulopatía recalcitrante, mientras que
el tratamiento quirúrgico definitivo primario
se indica en el paciente hemodinámicamen-
te estable y en ausencia de cualquiera de los
factores anteriores. (Szuba et al., 2019)
