necesario, que se haga exclusivamente
con maquinilla eléctrica con cabezal
desechable. El afeitado de la piel debe
ser evitado tanto en el preoperatorio
como en el quirófano. Muchos ensayos
clínicos han demostrado la efectividad
de no hacer rasurado previo (el mismo
día o en quirófano) de la zona de inci-
sión, que tiene una efectividad preventi-
va cercana al 50% de las IHQ. (Fustero
Montesa & Casbas Biarge, 2020)
4. Lavado de manos: La higiene de manos
es la medida de prevención más eficaz
y la más olvidada para evitar la transmi-
sión de microorganismos. Diversos estu-
dios demuestran que el cumplimiento de
la higiene de manos durante la atención
a los pacientes reduce la infección no-
socomial. El lavado quirúrgico de manos
es la medida de prevención cuya técnica
más ha cambiado en los últimos años. El
tiempo del lavado de manos quirúrgico
debe ser entre 2 y 6 min; más tiempo no
tiene ningún beneficio. En cuanto al tipo
de solución más adecuada, no existe
evidencia clara entre el uso de povidona
yodada al 7,5-10% o clorhexidina al 4%.

5. Técnica quirúrgica, tiempo quirúrgico
y reintervención: La mayor parte de las
infecciones quirúrgicas son el resultado
de la contaminación de la herida durante
la intervención. La habilidad del equipo
quirúrgico para minimizar la contamina-
ción bacteriana de la herida es crucial.
La duración excesiva de la intervención,
el trauma ocasionado, hemostasia inco-
rrecta, drenajes, tensión en las suturas,
isquemia, tejidos necróticos, creación de
hematomas o seromas, o falta de cumpli-
miento de las medidas de asepsia duran-
te la intervención son factores críticos que
contribuyen a la contaminación y apari-
ción de ISQ. La incisión de la piel con bis-
turí eléctrico aumenta significativamente
el riesgo de infección, por lo que debe
utilizarse el bisturí frío para la incisión dér-
mica. (Bravo-Coello et al., 2021)
6. Medio ambiente y circuitos de quiró-
fano: Se debe reducir la circulación,
actividades y número de personas en
las áreas quirúrgicas, puesto que el au-
mento de las mismas influye en el núme-
ro de microorganismos potencialmente
capaces de producir infecciones. Se tie-
nen que mantener cerradas las puertas
durante los procedimientos, el sistema
de ventilación funcionando todo el día
(incluso si no hay actividad) y según el
tipo de cirugía se establecerá los reque-
rimientos necesarios de temperatura,
humedad, número de renovaciones de
aire, presión, etc. No estarán permitidos
elementos de almacenaje de materiales
dentro del quirófano. Además, hay que
exigir al personal el uso de ropa limpia,
gorro y mascarilla. Se deben definir cla-
ramente los espacios de limpio (quirófa-
nos, antequirófanos y área de lavado de
manos), semilimpios (pasillos, zonas de
descanso, sala de reanimación) y sucios
(vestuarios, recepción de enfermos, sala
de información). Tras cada intervención
y al final de la jornada se establecerán
los protocolos de limpieza adecuados.

Los criterios de ISQ han variado amplia-
mente a lo largo del tiempo. Según la de-
finición clásica se consideraba una herida
quirúrgica infectada aquella que desarro-
llaba una colección de pus. En 1980, en el
proyecto sobre el estudio de la eficacia en
el control de infecciones nosocomiales, SE-
NIC, por sus siglas en inglés (Study on the
Efficacy of Nosocomial Infection Control) se
definió la infección de la herida quirúrgica
en base a los criterios de los centros para el
control y prevención de enfermedades de
los Estados Unidos, (CDC) para el National
Nosocomial Infeccions Surveillance System
(NNISS) como sigue:
